Evergreen Health and Rehabilitation Center received an overall 2-star rating out of 5 for the first quarter of 2026, the Centers for Medicare and Medicaid Services (CMS) reports. This falls below Virginia’s statewide nursing home average of 3.0 for the same period.

The facility had 176 available beds in the first quarter of 2026, 63 more than the average nursing home in Virginia. They provided care to an average of 171.1 residents during that time.

As of the first quarter of 2026, Evergreen Health and Rehabilitation Center faced a total of $8,190 in fines and was subject to one penalty.

Estimates from the U.S. Census Bureau indicate the number of older adults continues to climb nationwide, while younger populations decrease. From 2020 to 2024, Americans ages 65 and up rose by 13%, as those under 18 declined by 1.7%. The nation’s older adult population has expanded substantially since 1920, growing from 4.9 million to over 61 million in 2024.

A 2021 analysis found that nearly half of Americans in this age group—about 28 million people—may need long-term services and supports. The same study projects that more than a third could eventually require nursing home care.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
